The Healthcare Request
Community Nursing Service assists with:

 

  • Wound care: acute and chronic management
  • Medication management including IV medications for ACC and Non ACC conditions
  • Continence management including urinary and/or bowel function
  • Assessment of health status including blood pressure and assistance with diabetes management
  • Palliative Care
  • Referrals for Aged Care Assessment and other health care professionals
  • Education for clients, services and carers on health conditions including prevention, assessment and management plans
  • Commitment to nursing educational programme
